What’s the difference between a survey and a form? 

A survey collects the information into a PDF or an Excel file for you. It doesn’t email you the responses. You must go into the Survey area on the website and download it. 

A form emails you the information directly. It doesn’t save any information on the website. The information goes straight to your email only.

How to create a form

Forms differ from surveys in that the information is emailed to you and there is no back-end storage of the form. The email address that the form is sent to is the only record.  

Please let CME know what page you'd like as a form either by email or your project. Since a form page isn't the 'default' layout our website uses, we'll just need to adjust some settings in the website backend before you can build your page out. Once you've let us know which pages you'd like as a form, adjusting these settings is a super speedy process so you can build out your form page in no time! 




How to create a survey

Our website uses widgets to add various tools (like surveys) to your page. You’ll learn how to make a widget, build a survey and set up email notifications. This video goes a little further in-depth in how to access survey notifications and alter the notifications sent to your department or the user who has submitted the form. 

Language tip:  

Response notification — Sent to a specified address when a response is completed (the email that your department receives to say that someone has submitted a survey or form). 

Response acknowledgment — Sent to the respondent when they complete their response (the email the user receives after they have clicked the submit button). Note: this only applies to non-anonymous surveys.

Single - User has to be logged in to see the survey and can only do one response 

Editable - User has to be logged in and can edit their response after submitting 

Multiple - User has to be logged in and can do multiple submissions 

MultipleWithAnonymous - Anyone can do survey multiple times. 


What happens when someone submits a survey response? 


This module will teach you how to access survey responses. Your department will receive an email notification whenever your survey receives a response. These can be viewed and downloaded via the Survey Admin section of the website.
